Paper Registration

2010 Camper Health Form -[Health forms are good for two years.]

2010 Hold Harmless Agreement -[Hold Harmless forms are good for two years.]

Scholarship Application -Scholarship money is avaible for those who qualify.
Please send in your application no later that May 1.

All campers are required to have a completed health form on file to partcipate in camp activities.
In the past we have required a two-part health form. This year only one form per participant
is required. Health forms are good for two years. This means that if your child filled out a
form last year, then a new form is not needed. We are more than willing to check our files
to see if your child has a current form. Please contact our office if you have any questions.
